I want to update
our membership on what NCMGM has been doing!
o At the
National MGMA conference last fall, NCMGM and Larrie Dawkins (Chair of
the Davis-Watson Scholarship committee) were awarded a Meritorious
Service Award. The Board of Directors of the
American
College
of Medical Practice Executives (ACMPE), MGMA, and the ACMPE Scholarship
Fund presented the award. This award recognizes the membership’s
commitment to provide scholarship assistance to students through the
Davis-Watson Scholarship Fund. Noteworthy: this is the first time an
organization has been given this Meritorious Service Award! Thanks to
all of the members of the scholarship committee.
o The
conference committee has been very busy getting a great conference lined
up for the spring. If you haven’t heard, it will be at the Marriott
Grande Dunes, Myrtle Beach, May 17-19. Our new meeting planner is Melissa Klingberg in our association office in Charlotte. The co-chairs for the
conference committee are
Laura Sanborn and
Linda Wooten.
o The NCMGM Board of Directors and Leadership Council (Committee Chairs) will
convene in February for their annual leadership retreat to “regroup and
refocus.” Although some issues never leave our top priority, we
will have a new facilitator, Will Latham, to lead us in developing our
second “three year Strategic Plan.”
o One of our goals for this year is to strengthen
the relationships between the state and local groups and among the local
groups themselves. If you are the president of your local group and I
have not called you, please call me or email me at
amyr@4gna.com. We all need to network; this
area of volunteerism is an excellent avenue to achieve this goal. We
want to identify and develop new leaders in our membership and the best
resource will be the members at the local level. In addition, we want to
expand our efforts in providing an avenue for local groups to network
about numerous issues such as dues, meeting locations, vendor
sponsorship, speakers, etc. We plan to provide a special roundtable
discussion during our conference so the elected, local leaders can
discuss pressing issues common to all.
o We are
pleased and excited to announce that North Carolina, Georgia and South
Carolina are in the beginning phase of planning a Tri-State Conference
to be held in 2008. North Carolina was chosen to host the first
conference, with the other states hosting in 2011 and 2014. Pooling the
resources of three strong states will allow us to bring in the highest
quality speakers, offer specialty specific topics (which you’ve asked
for) and provide great networking opportunities to name a few benefits. We have
a great group representing North Carolina to ensure that this will kick
off the first of many successful conferences going forward.
o Of
course, our other committees continue to meet in an effort to improve
issues for all of our membership. Read on throughout this newsletter for
updates.
Here’s wishing you a very happy and prosperous New Year. Remember, take

Anesthesia-only practices appeared to cut costs where they could in 2004
and boosted their bottom line, according to the Medical Group Management
Association (MGMA) Cost Survey for
Anesthesia Practices: 2005 Report Based on 2004 Data.
Anesthesiology practices’ margin (median total medical revenue after
operating costs) grew 10.5 percent, from $403,029 in 2003 to $445,339 in
2004, despite a small increase in revenue. Median total medical revenue
grew 2.6 percent, from $509,141 in 2003 to $522,575 in 2004. While
anesthesia groups carry low overhead, the main source of savings
appeared as a result of staffing reductions. Interestingly, observed
groups reported lower levels of non-physician provider (NPP) staffing —
from 13.50 per group in 2003 to 10.10 in 2004 — but 8 percent higher NPP
costs.
"Anesthesiology has a shortage of physicians, so we pay a premium not
only to recruit associate-level physicians but also established
physicians and certified registered nurse anesthetists (CRNAs)," says
Asa Lockhart, MD, MGMA survey operations adviser and principal, Golden
Caduceus Consultants, Miami.
Since 2000, median compensation for CRNAs has grown 32.92 percent and
anesthesiologists' has grown 16.28 percent, according to MGMA’s most
recent physician compensation survey report. Lockhart also pointed to
the growing costs of professional liability in some markets and health
insurance benefits in all markets as factors in rising costs.
Because so many factors affect anesthesiology and pain management
practices’ finances and productivity, the devil is truly in the details
for this specialty. For example, staffing model has a major effect, with
2004 median total medical revenue after operating costs per
full-time-equivalent (FTE) physician in anesthesia-only practices at:

$379,102 for physician-only practices; |
|
• |
$424,294 for practices with less than one CRNA/anesthesiologist
assistant (AA) per FTE physician; and |
|
• |
$657,428 for practices with more than one CRNA/AA per FTE physician. |
Similarly, Lockhart
notes that practices providing pain services differ from anesthesia
practices, particularly in staffing requirements. The median total
support-staff cost per FTE physician for anesthesia-only practices is
$17,302, but for practices with more than 10 percent of cases devoted to
pain management, the cost jumps to $30,301.
"Pain is a paper-intensive specialty," says Lockhart. One anesthesiology
group he cited as an example has 0.14 FTE posting staff per FTE
physician for its operating room and obstetrics practice, compared with
0.6 FTE posting staff per FTE physician for its pain practice.
The investment may be worth it. Median revenue after operating cost per
FTE physician for practices with more than 10 percent of cases devoted
to pain management is about $50,000 more per FTE physician compared with
anesthesia-only practices, the MGMA data show.
The MGMA report provides data from more than 100 anesthesia and pain
management practices on staffing and operating cost, broken out per
physician, per anesthetic location, by number of trauma centers, by
staffing model and as a percentage of total medical revenue. New this
year is a section for pain management procedures.
This year MGMA conducted the survey in collaboration with the American
Society of Anesthesiologists.

The Medicare Committee held its last meeting on December 14, 2005 via a
teleconference format that was open to all NCMGM members. This was the
second year in a row that we had our December meeting in this format.
The Committee opted to open up our meeting in an effort to provide an
educational opportunity to other NCMGM members that are unable to travel
to
High Point for our
regular meetings.
Forty-six of our members from across the state registered to participate
and several others joined our host, Reshell Gardner from Cigna
Government Services, at their office in High Point.
Reshell answered
nine previously submitted questions as well as several others that were
asked during the meeting. Questions included properly billing for the
professional and technical components of ECGs to receive the HPSA PSA
payments, the requirement that all Medicare claims be submitted
electronically, and the change in the Eligibility-File Based Crossover
process. To read all the questions/answers from our meeting, please see

Member Links section of the NCMGM website (www.ncmgm.org).
In addition to answering the questions, Reshell included some very
helpful web links that provide further information on the topics
covered. I would like to thank Reshell and Crissy Strickland at the
NCMGM association offices for all their hard work in making the meeting
a success.

The two programs we are actively working with at this time
are UNC-Chapel Hill’s MHA students and the MGMA student chapter.
Eight NCMGM managers participated in a panel discussion with club
members on September 29 at
6PM. Wade Adams facilitated the discussion to answer
questions on such issues as scope of the participant’s organization,
salary ranges, what a manager’s typical day would be like and advice
each participant would give to a new manager.
Beth Davis is the current student chapter president and Oscar Aylor, FACHE,
is Clinical Assistant Professor, Director of Professional Development,
Dept. of Health Policy and Administration, UNC School of Public Health.
On October 19, three NCMGM managers held a panel discussion with
Appalachian State University (ASU) students in the Healthcare Executives
Club. Christina Young facilitated the discussion with an attendance of
about 20 students. Hunter Hawley, a senior at ASU is current club
president and the Director of the undergraduate program in Healthcare
management is Dr. Susan Roggenkamp. Dr. Roggenkamp recently established
a mentoring program that will pair a student with a manager. |

Both universities require internships for students, typically during
their last year of school. A newsflash went out December 2 asking for
manager’s interested in taking an intern for approximately 160 hours.
One other option requiring less time on the part of the manager would be
to offer a one-time shadowing opportunity, allowing a student to spend
one day at your practice to get an idea of all that is involved in
managing a practice.

Chip Baggett has
just become the new CEO with the Buncombe County Medical Society (BCMS).
BCMS has been instrumental in the statewide effort toward tort reform
and working with area practice managers, through their Advisory Council
of managers, to assist practices in reducing cost and manage more
effectively via cooperative purchasing arrangements and contracting as a
consortium of providers for various services, i.e., employee assistance.
Congratulations to Chip and thank you BCMS.
Tom Talley of Asheville Cardiology Associates in Asheville, NC, assisted
with the United Ways efforts in their annual fund drive this fall by
serving as the conduit for communication to the physicians practices in
the area. This was the first effort of this kind specifically
directed to the physician practices and it was very successful. It
provided an excellent venue to allow for employees and physicians to
give back to their communities.
Sherry Davis, the Practice Administrator for WNC OB/GYN & Family
Practice in Asheville, NC, recently passed her ACMPE boards to become a
CMPE. Congratulations Sherry!

Mutual Administrator of the Year Award. This award recognizes a
medical group practice administrator, affiliated with the North Carolina
Medical Group Managers (NCMGM), who has exhibited exceptional leadership
management proficiency and enhanced the effectiveness in the delivery of
health care in his/her practice and community through a recent,
noteworthy achievement.
